Ralph was born and raised in Wilmington and has lived in North Carolina all of his life. He graduated from New Hanover High School, then earned his bachelor's degree in Zoology from the University of North Carolina, Chapel Hill and his medical degree from Wake Forest University. He is married to his wife, Sandra, with whom he has 3 children and 9 grandchildren. He is Christian.

Ralph served in the military for 4 years. During his service, he was the chief of orthopedics at Torrejon Air Base.
